生物体对异种移植物或同种异体移植物常发生排斥作用，这是因为供体的基因编码受体所没有的抗原，被受体的免疫细胞识别后引起排斥反应；或者是移植物中的淋巴细胞识别了受体细胞的抗原，发生免疫应答，攻击受体细胞，产生移植物抗宿主反应(graft versus host reaction，GVHR)。上述两类都是因为组织抗原不同而引起了免疫应答，即组织不相容性(incompatibility)。如果移植物与受体的基因型完全相同，彼此都不视对方为异己，称为相容性(compatibility)抗原。组织相容性抗原分为“主要组织相容性抗原系统”(major histocompatibility antigen system)和“次要组织相容性系统”(minor histocompatibility antigen system)。前者指体细胞表面抗原能引起受体淋巴细胞强应答，产生强烈排斥反应；后者指引起弱应答和弱排斥反应。主要组织相容性抗原是一组抗原，由一组连锁基因编码。这组基因称为主要组织相容性复合体基因(major histocompatibility complex gene，MHC gene)。人的定位MHC在第6号染色体上，小鼠的则定位在第17号染色体上。

    组织相容性抗原按其免疫性能可分为三类：第一类是移植抗原(transplanta—tion antigen)，负责排斥外来的组织，存在于杀伤性T淋巴细胞(killer T leukocyte)中，为细胞介导的免疫反应所必需，小鼠的I类基因编码H2-K和H2-D／L蛋白质，人的I类基因编码移植抗原HLA-A，B，C；第二类是参与免疫反应过程中细胞之间的信息传递，人的Ⅱ类区称为HLA-D，又可分为4个亚区，排列顺序为DR，DQ，DZ／DO，DP；补体蛋白质提供了第三类MHC，基因座位于S区，其编码的蛋白 质是血清的组成成分，负责同抗原—抗体复合物作用以引起细胞的裂解。

    人的MHC基因座又称HLA，位于第6号染色体短臂，长约4 000kb。HLA—I区有A、B、C三个基因座，每个基因座又各自有几十到200多个等位基因。在I类区里还发现有正、F、G、H、J等基因座，称为I类样基因(class I—likegenes)。HLA—Ⅱ类区内有DQ、DR、DP基因座，每个基因座也分别有几个到100多个等位基因。HLA-Ⅲ类区里有补体C2、C4、Bf等基因座。HLA的各个基因座紧密连锁，位于同一条染色体上，就构成了单倍型或称单元型(haplotype)。例如，某一个体两条染色体上的HLA单元型是A1C1B1DlDR和A2C2B2D2DR2，它们分别来自父亲和母亲，这些基因是共显性，个体的表型就可视为基因型。由于HLA各个基因型都有很多等位基因，可以有无数种组合，因此除非是遗传因素完全相同的同卵孪生子，不同个体的HLA单倍型完全相同的概率极小。因为HLA与移植抗原有关，所以组织器官移植时，HLA的配型是否合适十分重要。

Just thought I'd spread the word of hope on what is hopefully a success with immune treatments.

 

I am pregnant, almost 5 weeks. I usually miscarry as soon as I get pregnant at 4-5 weeks and my HCG levels have never risen properly with me only ever getting faint positives on HPTs. Blood tests have only ever amounted to levels of about 50 or less so incredibly low.  

 

After 5 very early losses I read Dr Beers Book, Is Your Body Baby Friendly, and for that reason I got proper immune testing done:

 

These showed I needed LIT as my LAD test was negative. (In other words I was not producing anti-bodies against my husband to protect the fetus). I had this done 2 weeks before embryo transfer. 

I also had raised NK cells at 20% and have been taking 7mg prednisolone since 2 weeks before embryo transfer. Also had an intralipid infusion 4 weeks before transfer and another one as soon as tested positive at 4 weeks. 

Despite no known blood clotting issues I am on 75mg baby aspirin and 20mg clexane/lovenox per day. This is because the above problems can trigger blood clotting problems in pregnancy. Am also on double folic acid as husbands mother had a baby with spinabifida.

Also take fish oil with high DPA/EHA content. 

 

I have done 3 Clearblue Digital Conception Indicator pregnancy tests which tell you how far past conception you are. 5 days ago it said 1-2 weeks since conception, 2 days ago it said 2-3 weeks past conception and today the HCG has increased again with test saying 3+ weeks since conception. So HCG appears to be rising normally and have never had this before as my body has attacked every other pregnancy so early on that sometimes I wouldn't have known I was ever pregnant unless I'd tested before my period was due.  

 

It is early days yet but whatever happens I know my HCG is higher than it has ever been so these treatments have made a huge impact. So we are on the right track!

 

If you are suffering early miscarriages please please get an LAD test done - not many women seem to get tested for it and think their only problem is blood clotting or NK cells - but LAD can co-exist with these immune issues. Also, what is amazing is that my Auntie had exactly the same problem as me 22 years ago. She was one of the first women in the UK to have LIT as part of a research project in London. After 6 early miscarriages she had LIT and then had 2 beautiful daughters, now 21 and 19 years old! 

 

Fingers crossed I will hold on to this baby now that I've received immune treatment. If anyone wants any further info from me on the LIT etc please let me know and will try to help.

And in situations where people do not continue to IVIG treatments as

necessary (based on bloodwork results), sometimes they have to deliver the baby

early since fetal growth has slowed down so much? I am planning to continue to

test the NKs 10 days after each IVIG treatment to make sure they stay in line.

My RE has said that I will continue to get IVIG monthly at a minimum throughout

the pregnancy most likely.

So in the situations she's seen, where people stop doing IVIG as necessary,

the growth rate will have slowed enough that the docs will want to deliver the

baby - she mentioned 26 weeks as when that tends to happen (maybe because 24

weeks is viability). And of course that means months of NICU, if the baby

survives. I haven't asked specifically, but I think they'll have me keep testing

into the third trimester - 'cause they want you to get to 36 weeks at least.
